Recipient Responsibilities

Failure of a recipient to timely comply with his or her Recipient Responsibilities will result in loss of the scholarship.

Part One - General Responsibilities

  1. Must live on campus the first academic year unless waived by the Trustee of HSF.
  2. Must join one campus organization during first semester of Freshman year.
  3. Must maintain a Four (4) year graduation schedule.
  4. Must maintain a 3.00 cumulative grade point average during recipient's freshman and sophomore years and a 3.25 cumulative grade point average during recipient's junior and senior years.
  5. Must declare an academic major before start of recipient's junior year.
  6. Must not act in a manner which brings discredit upon HSF, its Grantor, Trustee, employees or volunteers, other scholarship recipients or the college or university attended by recipient, or himself or herself.
  7. Must apply for eligible Federal, State and Institutional grants.
  8. Must attend mandatory workshops and activities sponsored by HSF intended to help maximize each recipient's educational experience.

Part Two - Information to be provided to HSF

  1. Must email HSF of any change in recipient's name, email address, physical address or phone number. ALL HSF mailings will be mailed to the address provided on recipient’s Application until notified otherwise by recipient.
  2. Must provide HSF with a Release Authorization to the college or university attended by recipient, authorizing release of recipient information to HSF*.
  3. Must notify HSF of incurrence of ANY DEBT in excess of $2,000 subsequent to the award of a scholarship:
    1. Approved debt will not adversely affect recipient's scholarship.
    2. Unapproved debt will be deducted from the amount of the scholarship.
    3. Incurrence of Unreported debt will result in loss of the scholarship.
    4. Outstanding Debt will be confirmed each year by HSF.

The Purpose of debt notification is to ensure that a recipient does not incur unnecessary or burdensome debt while receiving funds from HSF.

Part Three - Reports to be provided to HSF
Each scholarship recipient must mail to HSF in one mailing by each date the following information.

First Mailing: By October 1 each year.

  1. After Freshman year. Recipient’s College Transcript showing courses taken, hours credited, grades received and cumulative GPA.
  2. Recipient’s updated Four Year Degree Plan certified to by the college counselor or registrar.
  3. Recipient's Employment Report documenting 240 hours work.*
  4. After Freshman year. Recipient's annual Progress Report documenting progress toward those personal goals represented to the Grant Committee and contained in recipient’s application.*
  5. List of ALL scholarships, grants, awards, and waivers received and applied for by recipient for the current academic year.*
  6. Recipient's updated FAFSA SAR (Student Aid Report), must provide the complete Report.
  7. Recipient's updated Financial Aid Award Letter (FAAL).
  8. Proof of Parents / Guardians payment of recipient's FAFSA Expected Family Contribution (EFC) for the current semester.*
  9. Recipient's Financial Report listing all sources of income and expenses for the prior academic year.*
  10. Recipient's Financial Budget listing all sources of income and expenses for the upcoming academic year.*
  11. Recipient's Financial Statement listing all assets and liabilities.*
  12. Recipient's must attend mandatory workshops.

After October 1, after recipient eligibility is confirmed, the award amount will be determined and disbursed to the Financial Aid Department of recipient’s school.

 

Second Mailing: By March 1 each year.

  1. Freshman year only. Recipient's confirmation of membership in a campus organization.*
  2. Recipient’s College Transcript showing courses taken, hours credited, grades received and cumulative GPA.
  3. List of ALL additional scholarships, grants, awards, and waivers received and applied for by recipient since recipient's last report.*
  4. Proof of Parents / Guardians payment of recipient's FAFSA Expected Family Contribution (EFC) for the current academic year.*
  5. Updated Financial Aid Award Letter (FAAL).

After March 1, after recipient eligibility is confirmed, the award amount will be determined and disbursed to the Financial Aid Department of recipient’s school.

 

Part Four - Mailings and Email

Mailing Instructions:

The envelope return address must first show recipient's assigned Identification Number, then Last / First Name, then recipients Name, Address, City, State and Zip Code.
